A Senior Placement Solution is here to provide you with assistance. Our process is as follows:

We begin with an initial client evaluation process. The evaluation process incorporates our understanding that each client has a different set of requirements. This process will help us understand what senior facility best meets the needs of each client.

Our evaluation services include:

• A medical history review, working with health providers if needed.



• An Alzheimer's Disease evaluation.



• An evaluation of activities of daily living (ADLs) which are evaluated to determine levels of care.



• A safety evaluation which includes a fall risk assessment, and all other safety issues.

Once you have choosen the facility, we will work together with the facility nurse to ensure that a care plan is in place prior to move in.

Within 90 days of placement we do a follow-up call to ensure that the new retirement/assisted living/adult family home community is to your satisfaction. Once a client is placed we will assist at anytime if higher level care is needed.
